This is an exciting time to join us. The three NHS trusts that currently run Broomfield, Southend and Basildon hospitals are planning to become a new single NHS organisation by mid-2019.
All three hospitals will continue to provide the services our patients use the most. These include outpatient appointments, children’s services and day-case surgery. All three hospitals will continue to provide 24/7 A&E and maternity services.
By working as one team we will provide the same high standard of care and services for all of our 1.2m local residents. Using the same equipment, processes and IT systems means services will run more smoothly. Single leadership, standards and new structures will speed up decision making. Combined corporate services will reduce duplication and cut running costs. This means more money for front-line services and faster improvements.

We are currently recruiting for a dynamic individual to work as a part of the assessment team for Hospital 2 Home, contributing to screening patients on the wards and developing care plans and protocols based on individual needs.
The role includes facilitating the discharge of Complex patients in short stay areas, supported by Local Authorities and the complex case management team. The successful candidate will undertake specific duties involving direct and indirect care as identified by the multi professional team to support the delivery of effective discharge planning to patients/ service users.
The ideal candidate will have experience of co-ordinating, assessing and implementing care plans, knowledge of Physiotherapy and Occupational Therapy as well as experience of delivering patient care in the health care setting/ community or nursing home environment.

You must have appropriate UK professional registration.
